In the office doc injected Right L2/3, Right L3/4, Left L4/5 and Left L5/S1. So none of these are bilateral and 64495 can't be billed twice on the same day. So I bill 64493 RT, 64494 RT, and 64495 LT and the inj med but there's no way to get 4 injections billed when none of them are bilateral at the same level. Right? The 4th one just gets included in the 64495. I've done these for 3 years and I think this is the first time I've seem him do this and it's throwing me off.
