66170 versus 66172- I can argue back and forth with myself all day long about which is correct but I'd like some other minds participating. Which CPT do you use when the surgeon doesn't document scarring from a previous surgery but per patient's H&P you know they've had surgery in the operative eye previously? Of course, ideally that would be documented if it is seen but here we are... One code states "in absence of previous surgery" while the other clearly states "with scarring from previous surgery or trauma".

Physician query possibly, they really need to document properly for the correct code selection. You cant assume based on past surgical history.
