When billing post op management servicers should the dx code from the surgery be the dx code used for the post op visit?

you do NOT use the dx from the surgery, that condition does not exits anymore, you use a V code for either follow up or aftercare. Just because you received payment is no indicator of correct coding. If you performed a cataract removal then there is no more cataract in that eye.
 
I have a ins. that does not like the V codes saying due to NCD. I thought the same thing, the cataract does not exist.
 
The V code is correct. You will need to convince them of that, To say it is due to NCD makes no sense since it IS medically necessary to follow up from a surgical encounter. We do not code to satisfy a payers wants. We code for the documentation provided.
