67830 - Correction of trichiasis by incision

Medicare has an MUE of 1 on CPT 67830 but it can be done on each eyelid. Would I list each eyelid on a separate line the the E1, E2, E3 or E4 Modifier? I have a patient that had all four eyelids treated.
