The office I work for is currently having denials on some of our orbits x-rays prior to MRI. We are screening for metal prior to the MRI. We use 70030 with dx code T15.80XA. When we researched this further, we are not sure on somethings.
  1. Should we be using a different diagnosis for the orbits screening prior to the MRI?
  2. With modifiers, do we bill with a 50, or two line items with RT/LT mod?
  3. When we get the images, it is of both eyes, not just one side, wondering if 70030 is the correct CPT in this case?

I have recently been advised to use a modifier 50 unless the order or documentation is specific to one eye and also to use Z57.5 if the patient has a history of welding or working with metals. We are a rural health clinic and most of our patients are farmers and ranchers, so that is ususally what we end up using for DX. Even high school students taking metal classes or auto body classes should probably have the XRay performed on both eyes. If anyone else has more information, please let us know.
