Per CCI edits, modifier use may be allowed for separate payment but, can someone tell me when it is appropriate to code 75724-59 when used with 93458.
In this case the indications were; angina and uncontrolled hypertension. Per the angiographal data; the LAD, LCX and RCA revealed 50% to 70% stenosis. Per the peripheral angiogram, the left and right arteries were viewed. The interpretation from the hemodynamic data is left side systemic hypertension.

It is not appropriate to use 75724 with 93458. 75724 is a selective code meaning that a catheter must be placed in the vessel to use it. (renal).
