76 modifier in the CPT Code book states repeat procedure or service by same physician or qualified health care professional

77 is repeat procedure by another physican or qualified health care professional.

We have physicans in our practice who read EKG's for a local hospital. I thought the 76 modifier signified that the same referring Dr requested multiple EKG on the same day and if two different referring physicans requested an EKG on the same day you would use the 77. Is this correct? Or would the 76/77 modifier refer to the same doctor in our practice reading the EKG
 
76/77 refers to whether the repeat procedure was PERFORMED by same or different doctor. It has nothing to do with who requested it.
