If a patient has a CT of the Head and then of the Chest, Abdomen and Pelvis with 3D reconstruction performed on all areas...........how many time can you submit the 3D reconstruction code? Can you submit this for each area with a 59 modifier? Where do I find the supporting documentation for multiple charges?
