Hello All!

I have a doctor that wants to bill a 76499 for the reading of a cholangiogram while doing a lap chole. In his op note, he always says something like:

"A choloangiogram was then preformed using the Kumar clamp system. Contrast was noted to flow down a tortuous dilated cystic duct to fill an extremely dilated common bile duct. Along the distal 1 to 2cm of the common bile duct, the luminal caliber of the bile duct narrowed significantly..... The Kumar clamp was then removed."

According to the insurance, they need more information to pay for 76499. What other information can be included to get this paid by insurance?
