When a provider does an ultrasound (76881/76882) and an injection during the same visit, would you place the 59 modifier on the ultrasound code or the injection code? Or one on both? I keep getting mixed answers on this.

76942-59 (Ultrasonic guidance for needle placement (eg, biopsy, aspiration, injection, localization device), imaging supervision and interpretation). We do not use 76881/2.
