Wiki 76937 for fistulogram

Hi,

I have an clarification on CPT 76937 with fistulogram and Interventions ( 36147, 37205, 35476
). Is 76937 included in the above mentioned codes or need to code separately ?

Can anyone explain in detail what are the documentation needed in the report to bill for 76937 . Please give me the rationale if it is inclusive in the above mentioned codes.?

If access is the shunt directly, then S&I is bundled. If access is not directly, and the shunt is imaged, then 75791 is coded.

76937 ultrasound guidance is covered with 36147. If you check the cpt professional edition (ama) page 205. Ultrasound guidance for puncture of the av shunt is not included in 36147. Must dictate cannulated using ultrasound.
