77012 No Seperate Payment, do you include on claim form for medicare?

77012 is on the ASC list, but with no seperate payment. My question is do you include this on your claim to Medicare with TC & GY modifier, or leave off the claim all together. Will there be a problem when the physician bills 77012 26?
