:confused:Our surgeon does hand surgery in addition to plastics. We have our own x-ray machine and the doctor usually performs a joint survey x-ray in two views so we bill out 77077 twice. Lately the code has been denied by all insurance companies. Is anyone else having this issue? There is no LCD and I can't seem to find any documentation stating why it keeps being denied. We've tried to bill it with -59 and -76 and tried on one line with 2 units.
