Wiki 88305 denying claim due to excludes 1 Note

Receiving several denials on billing 88305x2- DX D18.01 and D22.5 ( D18.01 has excludes 1 note)

Appealed claim stating these are unrelated dx and unrelated lesions AND appeals are being upheld

does the 88305 have mod 59 it needs one maybe appeal again but state they are separate lesions
