Hi Can anyone tell me what I can do about billing 88342 x4, Aetna denied it stating that it is "This code is not separately payable according to incidental/mutually excluse CMS NCCI edits." They only paid for 1 unit, so I then sent a corrected claim, 88342 x3 and 88342 x1 with a 59 modifier, then they took back the one 88342 that they paid, and they aren't paying the other units and I have called Aetna at least twice to explain that per Medicare 3 units can be billed and paid separately. Am I coding this wrong? and how would you appeal this?
