I discovered our pediatricians are not billing the 90460 or 90461 when a child receives immunizations.

The following is the documentation and I believe it is adequated to support the counseling requirement:
"Procedure risk and benefits, indications, purpose, complications, alternative options, we discused with the patient;parent signed consent form.

If anyone has better documentation suggestions, I would love to hear them. I just want to have a second opinion before I tell the docs they are missing revenue.

They are missing alot of revenue (if they are providing the counseling).
Our m.d.'s will use something as simple as all vaccine components discussed with parent. Parent agreed to administration. Vis provided. Ljb
