Wiki 90801 guidelines

The documentation guidelines for 90801 state that there needs to be a complete medical and psychiatric history including past, family and social. Can we just have the patient fill out a history form and then document the provider reviewed it? This is being debated in our office any help would be appreciated.

In our office the providers scan that form into the electronic medical record and then they will reference it and code the 90801.
