Please advise, patient comes in early in the day with an eye exam 92012 using a glaucoma dx code. Now, the unfortunate luck of the patient, he had to return because whatever the patient did between the time of his earlier visit, he had to return to the office for a chemical burn of the eye from the peroxide solution he mistakenly used. That would be another dx code separate from his earlier visit and it's a new problem.

Could we bill out an eye exam and E/M. There are 2 different dx codes, plus adding a modifier 25 on 92012? Is it possible?

Sorry, I missed the 25 mod part...if a procedure was performed and the exam was done to determine that then no exam is billed. Just the procedure. The first exam with the different diagnosis would have a 25.
