Question: if an Initial 92072 fit has been initiated, and the first lens needs to be adjusted for BC or power, is that considered a new fitting or still part of the initial fit?

Additionally, with respect to routine contact lens examinations (92310), I would like clarification regarding billing procedures for the second and third contact lens fitting follow‑up visits. Is it appropriate to bill code 92012 for these follow‑ups? If not, which code should be used? This aspect has been a persistent point of confusion.
At present, we assign differentiated pricing to 92310—including follow‑up visits listed at $0.00. However, we are observing that many insurance plans require copayments for each office visit, including follow‑ups.
Could you advise on an appropriate, universally accepted code that should be billed for these follow‑up visits in conjunction with 92310?
