Hello! The February 2011 CPT Assistant states that it is not appropriate4 to report both 92133 and 92134 at the same patient encounter. Why is that?

92133 and 92134 are considered mutually exclusive and cannot be billed together regardless of diagnosis and not modifiable..... why....??? There is no explanation I can find other that what I have given here.
