Good morning,

I need some clarification regarding the documentation for the 92136-26. The patient is seen in April and a global 92136 is billed; surgery left eye follows. Then in the May the patient is complaining of blurriness of the right eye and wants cataract surgery. The physician bills 92136-26.... doesn't there need to be some sort of dictation stating he reviewed the 92136?

Yes, there should be something in the note stating he reviewed the test.

Also make sure you add modifier 26 and which eye was done. Example: 92136-26,RT
