I am getting denial for EKG 93010 stating it needs a modifier. I had initially thought mod 76, if there was another EKG done the same day, but after reviewing the notes, I only see the one EKG done for dos. Patient was also seen by same physician who requested the EKG for inpatient sub visit on same day. So would I code mod 25.
Suggestions?

My suspicion is that, depending on the insurance, they are looking for modifier 26 even though the 93010 is for the interpretation and report. The modifier 25 would go on the E/M visit and not the EKG.
