Has anyone gotten denials for 93228 inconsistent w/ POS? These are remote External mobile cardiovascular telemetry with electrocardiographic recording. Office/11 POS is used but it denials.

Also, how does one of these get paid "remote" reading if the patient is in the hospital (inpatient) and office bills POS 11?
