Does the documentation for 93351 have to state "continuous ecg monitoring"?

yes, to my understanding you have to have a retrievable continuous EKG just like a regular stress test

So on our template, it has a place for rest ecg and stress ecg, where the physician notes variables, etc. Is that enough IF there is a "retrievable" continuous ecg? That part is indeed housed with the procedure, but the template does not state specifically "continuous", even though it is.

Yes, that's fine. A physician has to have a continuous EKG to be able to interpet a stress test. Once they've done their report you don't even need the continous EKG anymore except to be able to retrieve it in the future in case there's any question of it being done or any question about the physician's interprettation. hope this helps
