My doctor billed 93351 in a DO place of service, but is being denied. Is modifier [26] required?
 
Does he own the equipment? My physicians do both; in office and outpt. We don't bill with 26 in office because we own the equipment. The ones we do outpatient are referrals from other physicians.

What was the reason for denial? That might help determine course of action and possible solution.
