when i bill 93454 do i need to use a 26 modifier with 93458 billed in the hospital

i was told that you can bill them together as long as 93454 was selected and the dr has written something and noted everything
is that true??? thanks:)
 
Whomever told you this was incorrect. 93458 is a 93454 with left heart catheterization with or without left ventriculography. In other words 93454 is included in 93458 and not separately billable.
