Medicare is denying 93571[26/LD] as an add-on code that shouldn't be billed with 93458-26. Both codes are using the same Dx 411.1. Is this correct?
 
Not according the NCCI edits from Medicare. See below: (I c&p from Mcare's NCCI link)

I would appeal that with records since NCCI does not show any bundling edits.
