My physician is wanting to bill 93657x2 for continuous lesion lines covering the roof, mid, and floor of the posterior wall when done after afib ablation. Do you agree with my physician with billing 93657x2? Thanks!
 
It truly depends on the wording in the op note. If a box wall lesion (roof and floor line) is performed and described as such, only one 93657 is billable. If it is not the intention beforehand to complete a box lesion, and one line is performed, but then it is decided that an additional line is needed, then technically you can bill 93657x2
