Wiki 93784 Ambulatory blood pressure monitor

My question is, should a nurse visit 99211 be billed with the 93784 ambulatory blood pressure monitor. My personal feeling is the application and removal of the monitor is included in the fee for the monitor. The nurse applying it to the patient doesn't do anything extra, so why would a level 1 visit be charged with it. The other coder in my office feels the nurse visit should be billed, I disagree. Any thoughts out there
