I am having issue with receiving payment for CPT 93866 from all payers. Provider is documenting I60.9. Can anyone help

Without knowing the denial reason, I can only guess it is due to the unspecified diagnosis I60.9. Payors are cracking down on the use of unspecified diagnoses. Review documentation to see if there is a more specific diagnosis, such as the location of the hemorrhage I60.0-I60.8.
