We have suddenly been receiving denials from Cal-Optima when 76856 and 76830 are billed with 93975. They used to pay them and our doctors are documenting appropriately, and the modifier 59 is being utilized. Does anyone have any insight as to why Cal-Op would suddenly start just kicking these out as inclusive?
