I find differing information out there regarding whether you can bill 94640 and 94664 on the same day. And if you do, would we append a modifier? Can't open the NCCI edit on this computer and getting annoyed today.

And while we are at it, 94760 Vs. 94761. It would seem to me that if you bill a 94761, you should only be able to bill 1 per DOS since it is for multiple determinations. Does this make sense?
