We have been billing 94664 with E/M code but it keeps getting denied. All of the articles I can find on this say to append mod 25 to E/M code, which we are already doing, the E/M code is paid but 94664 is not. Any help on this?
 
Who is billing this code - the physician or a respiratory tech? That could be where your problem is. It is a physician code only. There is a different code if your respiratory person is doing this service.
If this doesn't answer your question you may have to call the payer.
