Wiki 95 or 97 documentation guidelines

I work for a urologist and am confused about the difference in the guidelines, I just wanted to know what the majority of providers are using. We just upgraded our system to a newer version and there are differences in how the doctors document and I want to make sure we are using the best guidelines for our practice. Any information would be greatly appreciated. Thank you.
 
Our group was only using the 1997 guidelines when I first came onboard as the coder. After failing to meet documentation guidelines for the exam portion in several areas, the providers subsequently chose to use either the 1995 and the 1997 guidelines, whichever happens to be most beneficial.
