New Pt level 3 office visit 99203...Detailed Exam, "1995 Exam: Need 2-7 body areas and/ or organ systems with at least 1 being of a detailed nature". Doc notes Constitutional, Cardiovascular, Muscleskeletal, & Neuro. 4 elements. The Muscleskeletal is "detailed" in nature. Does this then constitute a Detail Exam with only 4 elements? Any thoughts is appreciated. Thanks. :)

I am finding that different contractors want the add on code 11045 (add on code for ea. add'l sq cm debride) billed in different formats. Can someone comment on this and if they have come across this issue? Contractor's are: Novitas and FCSO.

Ex. Total debrided:82.3 sq cm.
