Yes it has been deleted as of the 2020 CPT year. There are instructions:

In the future, you might want to post this in the PT forum as there will be more people who know answers to your questions than in the Medicare forum. To tell the truth, your question is not a Medicare question, it is a CPT question. I am glad I was able to answer it since I am not a PT expert.
