how to bill both services, 95873,95874 with 64614, they are both add ons and only one 64614 was performed cant bill these two together 95873,95874, but they are both performed?

Could you post a de-identifed note? The only scenario I can think of is that they used 95873 for muscles of the trunk and 95874 for muscles of an extremity?? Since 64614 is only reported one time, regardless of the number of muscles? But the doctors here usually use either or, not both at one session.
