Wiki 95957 Documentation Requirements

Does anyone know what is required to document CPT 95957 - Digital analysis of EEG? Most insurances are now requiring notes for this procedure, and are denying, stating that it is not documented. BCBS's policy bundles it regardless of documentation. Our doctor is an neurology epilepsy specialist, and is insisting that he does perform this additional analysis service, but is questioning what is acceptable documentation. Does anyone else bill this service and have sample documentation?

So the basics of documentation are generally the same. I will post some guidance. However, since this CPT can be either Technical(TC mod) or Professional (26 mod). For the Technical component I am not certain, for the professional component there will need to be a separate written report that has the Findings, similar to an EGK or X-ray report.
