Child is being seen for preventative visit, ASQ is filled out and scored on the last page. Is the Information Summary on the last page of the ASQ form enough to support the report requirement? Doctor doesn't document any discussion with the parents or anything else.

Always remember, if it isn't documented, it wasn't done, according to CMS.
A summary may not support medical necessity. I would inquire with the payer you are submitting the claim to.
