96116,AH, 96130,AH, 96136,AH Denial?

Medicare has denied 96116 stating missing/incomplete/invalid HCPCS. Can 96116 be billed with these codes or has something changed? Any help is appreciated as I do not do a lot of of BH billing. Thank you!
