When coding for chemotherapy and the initial service is 96146 {initiation of prolonged chemotherapy infusion (more than 8 hours), requiring use use of a portable or implantable pump} and you can only have one initial service. How do you code for the subsequent codes 96415 and 96411 when, (per CPT guidelines) they both require the use of 96413 as the initial??? Can someone help me with this one.

Would you bill 96146, 96413 and 96411 + the drugs?
