96402 done twice on same day

Our company has picked up billing for an Urologist and it is all new to me!

The urologist has given me the following procedure codes for a single date of service; 99212, 96402, 96402 and J9999.

My question on the above is: can I bill 96402 twice and do I need a modifier--possibly 76 or 59?
