We are receiving Medicare denial 109 incorrect payor when we bill 96523 and were told by MC that for PB in pos 21/22 we need to be adding modifier 26 to the claim. Modifier 26 appears to be one you cannot use with this cpt code. Has anyone else encountered this? How did you correct this issue?
