Are you required to do an evaluation on a patient (97162) not seen in therapy before and wants to charge a 97760? The initial order from the doctor was for orthotics only and not evaluation and treat. Two further questions:
1. If so, what is the time and documentation requirements to justify both.
2. And/Or in other words can you just bill 97760 by itself?
