Our Provider Organization wants us to bill these to satisfy PCHM. I am looking for an example of what the documentation looks like. If an RN does the calls, we bill under the Supervising Provider, does he need to sign off on them? Everything I read says the call has to be initiated by the patient, our PO wants us to call our BCBS/BCN Medicare patients, if we are doing this it is not Patient Initiated, correct? Any guidance would be appreciated.
