Wiki 98975 denials same day as E/M

We began receiving denials for 98975 billed with an E/M even when there is a mod 25 on the E/M. Per CCI 98975 would be included but a modifier is acceptable. Anyone have issues with RTM codes (98975, 98976) being billed the same day as an E/M? Since RTM is every 30 days (with at least 16 days met) it does has the possibility of landing on a date that the patient has an E/M service.
