99000 Does anybody our there use this code to bill for Handling and/or conveyance of specimen for transfer from the office to a laboratory in a CAH Rural or other Hospital setting for sending tissue biopy specimens out of town?

Is this specifically for our rural situation and can be used for all biopsies sent out?

Does this include the entire package per patient encounter or is this each specimen?
