Can a hospital charge out a 99211 or can this only be used in a clinic/office setting for a nursing visit such as dressing change, suture removal?
 
A hospital outpatient facility charges the visit level based on their facility specific criteria for that level. SO yes a facility can charge a 99211 for any service that meets their stated criteria for a 99211. The facility does not use the physician criteria for their visit levels. The facility criteria is based on the utilization of facility resources.
