Wiki 99211 Non Face to Face - Billing Error

I just finished an arugement with my doctor's billing staff that it is standard practice that they bill 99211 with each and every UA that is performed. I never saw the doctor, RN, or MA, just the receptionist. The argument was that it does not state that face to face is required to bill 99211 and that they bill this regardless. It also does not meet the requirements of "incident-to".

Of course, I know that a face to face encounter is required and there is a lot of documentation out there that supports this. Does anyone have anything specific links they could provide so I can provide additional information to support my argument?
