I'm getting denials from BCBS saying that the Rendering provider isn't eligible to perform the service billed. I'm unaware of anyway to signify that this was done by a Nurse. Any suggestions? This is for a 99211.
 
A 99211 performed by a nurse (RN/LPN) incident to a physician for carriers that accept Medicare's incident-to guidelines should be billed under the physician.
Were there any other services rendered?
Was the claim submitted under the physician?

Send the nurse note/ documentation of what the nurse did to the insurance company and appeal the denial. Anything done in care of the patient would need to be documented.
